1. Fill out the Application Form.(Omit this one if you apply online.)
2. Rabies vaccination certificate issued by a vet clinic of the exporting country.
3. Copy of the applicant's passport.
4. For imports from rabies-infected areas only:Report of rabies neutralizing antibody titre test issued by a rabies reference laboratories of OIE or a laboratory designated by APHIA, specifying the microchip number of the animals and the titre no less than 0.5 IU/ml (blood samples should be taken between 90 days and 1 years prior to shipment).
To make the application online, documents 2, 3 and 4 should be saved as .pdf, .jpg, .png or .tif files and uploaded to the online application system.
